YCCHS is required to complete the assessment every five eyars, and the last assessment was completed in 2012.”

The purpose is to identify pressing health concerns and and needs by collecting information and data from the community. The assessment will be completed between March and June, 2017.

The survey is available in both English and Spanish. All Yavapai County residents are encouraged to take the survey, participate in a focus group or be interviewed. There are 19 questions, and should take no longer than 10 minutes.
